Electronic CVI Basics

This system was designed to replace the paper form of the Certificate of Veterinary Inspection and provide faster notification of animal movements to the Tennessee Department of Agriculture and to the State's of Destination.
The CVI has been broken down into several parts in the form of Tabs.  This design allowed the designers to take a very busy and complex form and break it down into small manageable parts that are easier for the persons entering the data.
As of July 2011 this system has been tested and optimized to run in Internet Explorer 8. 

 
Sign in Page
 

An important issue concerning any system is security.  One way of providing security is via a secure site which this system uses.  Another way of providing security is to have and ID and password system in place for it's users.   By using your TN Department of Agriculture assigned Veterinary ID with password an additional level of protection is provided.
This information allows the CVI system to validate every user even on their first time to enter the system.
Thus on the Sign in Page, enter this information into the Veterinary ID and Password boxes.  Pressing the [Continue] button will submit this information which is then verified against the secure in-house system.  If a match is found you will be advanced to the actual on-line CVI electronic forms.

Signors Tab

This tab holds information concerning the buyers and sellers.  Additionally the reason for the certification 'CERTIFICATE ISSUED FOR' and PERMIT NUMBER are entered on this tab.  

Important Notes:
 

CERTIFICATE ISSUED FOR contains 4 choices.  If you choose Feeding the Animals tab will be formatted to accept BATCH animals.  Otherwise the Animals tab will be formatted for you to enter information for each individual animal covered in the Certificate.

PERMIT NUMBER is a required field.  But not all Certificates require a permit number.  If this is the case please simply enter N/A in this field.

First Name, Last Name, Business Name.  It is valid to only put in the business name.  But if a First or Last name is given, both are required.
Additionally it is valid to only give the first and last names leaving the business name blank.

Notes about Date fields: 
Date fields are set up to check for improper dates such as 2/31/12.  Additionally the system attempts to fill in the /'s for you as you type.  For Month and Date it is looking for two digits after which it will put in the /.  For example let's say you're keying in August 5th 2012.  If you type 08  the / will automatically appear and you are ready for the 5.  If you key in 05 again the / will automatically appear.  Or you can simply key in 5/ followed by 2012 or simply 12.  So if you key in 8/5/12 this is valid.  Additionally as you exit the field the system will change the format to be 08/05/2012.

Status/Tests Tab


Only two fields are always required on this tab.  The Species and the number of Animals in the Shipment.  But if additional information is given it must be complete.  For example you cannot mark Accredited TB without giving the number of animals and the last test date.

The number of animals will be used in the Animals tabs.  For example: if you have 10 animals that are listed as Certificate Issued for SALE, there will be 10 rows listed under the Animals tab.  Or if Certificate Issued for is FEEDING the batch animals screen will be expecting a total of 10 animals be listed.
 

Animals Tab

Two types of screens will appear in the animals tab based upon the choice given for CERTIFICATE ISSUED FOR in the Status/Tests tab.

For choices Sale, Breeding and Other there will be a row for each animal.  At a minimum you must give the Official ID, Age, Breed and Sex of each animal.  If 33 animals are involved there will be 33 rows to be filled out.

For the choice Feeding there will be a screen with six rows.  At a minimum at least the first row must be filled out.  All columns in a row must be completed.  
Examples would be:  
14 animals, 6 Male Red Angus, 6 months at an average weight of 200 pounds and 8 Female, Red Angus, 6 month at 190 pounds.
This would be entered as:
6    6m     AR   Male     200
8    6m     AR   Female  190

Electronic Signature Tab

A seven character alfa numeric electronic signature is generated for each CVI transaction.  This along with the E number listed on the final printed form is stored with the Veterinary ID.  This provides another level of security and enables the retrieval of CVI information in the future.
This Electronic Signature string is also printed on the actual CVI to provide additional verification as to the authenticity of the form.
 

Finish Page

This page has four options:
[ Preview CVI Form ] - This provides a preview of what the actual CVI form will look like.  It's main purpose is to allow an easy way to check for errors with everything being in one place.
[ Edit CVI Form ] - This puts you back on the Signors tab and allows you to go through all the tabs again to make any needed changes.
   Note that this option will not allow you the change the choice for CERTIFICATE ISSUED FOR.
[ Submit ] - Sends the information to the Department of Agriculture and allows the printing of the final CVI form.  Note that you will be allowed to print multiple copies of the form.
[ Return to Department of Agriculture Page ]  Aborts this system and returns you to the Department of Agriculture's home page.

CVI Print Setup

Note that this system has be optimized for use with Microsoft's Internet Explorer.
To properly print a CVI there are a few things that will need to be done to assure that the margins and boards are correct.   This will involve using Internet Explorer and the menu at the top.
It should look something like the following:

Click on the File menu choice and select the option for Page Setup...
You should then get an input window that looks something like the following:

Set your input window to:
Paper Size:  Letter
Landscape
Set all the Margins to 0.2
Then set all the Header and Footer pick lists to -Empty-
Then Press the [OK] button to save the changes.
Your CVIs should now print correctly.

Printing a CVI

Start by pressing the button [ Print CVI Form ].  Upon doing this the actual form should come up in it's own window.
It is suggested that you mouse in the area of the title which is CERTIFICATE OF VETERINARY INSPECTION.
Right click the mouse and select the option for  Print Preview...  Make sure the view is set to Landscape. And press the printer icon.
Depending on your operation you may get a dialog box asking for which printer to use.  If needed selected the desired printer and then press the
[ Print ] button.
